Accuracy of
new blood tests

One tool is
a blood test.

There’s not
always been a test for everything out there.

However,
there are a few new pathology exams (and more on the horizon) that can help
doctors better detect diseases and conditions.

Here are
three recent developments:

Viral vs. bacterial infections: Wouldn’t it be great to know if you
really need an antibiotic or if you should just let a cold run its course?

A Duke
University team is looking into the efficacy of a test that could distinguish
viral from bacterial infections.

Dr. Geoffrey S. Ginsburg previously told Healthline that
he would love to have the “pregnancy test equivalent to viral infections.”

Heart attack: Researchers at King’s College London created a blood
test
that better detects heart muscle damage due to a heart attack.

When
a doctor suspects that a patient has had an attack, only some incidents show
changes on a heart trace or ECG. Those exams test levels of cardiac troponin,
which is a protein released by the heart that is common after heart attacks or
inflammation. When doctors can’t find cardiac troponin in the blood, patients
are labeled as low-risk and typically discharged.

Researchers,
however, looked at 4,000 patients and found that 47 percent of those actually
had an intermediate risk and required more tests. Their test for myosin-binding
protein C is a better indicator of how much heart muscle has been damaged.

Dr.
Tom Kaier, a cardiologist with Kings College London, told Healthline that the
myosin-binding protein C is present in greater abundance and may rise quicker than troponin
after a heart attack. That could change the way heart attacks are diagnosed.

“Troponin is
an excellent test, but it has its limitations. It takes over three hours to
appear in adequate levels in the bloodstream to be measured reliably,” Kaier
noted.

Considering that only 14 to 17 percent of patients that go
to emergency rooms with chest pain have a heart attack, the other 80 percent
want reassurance as soon as possible. A quicker diagnosis may also improve
patient outcomes.

The detection of myosin-binding protein C “appears in the
circulation quicker, and thus could rule out heart attacks in more patients at
an earlier time point than currently possible,” Kaier added.

Concussions/traumatic
brain injuries:
Accurately diagnosing
a concussion or traumatic brain injury can be a bit of a guessing game when it
comes to specifying the severity of the impairment.

But Dr. Linda Papa, an emergency medicine
physician and researcher at Orlando Health released a study in JAMA Neurology
detailing a blood test that measures a protein released by brain cells upon an injury.

More of the
protein indicates a higher severity of brain injury, she explained.

Symptoms of a brain injury can be subtle and may
not come on for several days. She told Healthline that
diagnosing a concussion can be “very subjective” and is “often based on the
experience of the clinician.”

If a person doesn’t see a brain expert, they may not be
accurately diagnosed, so having a concrete test to confirm injury would be a
big step. A CAT scan can give an assessment of a brain injury, but it only
detects lesions and cannot detect all of them.

Having a definitive blood test could help doctors
make better treatment decisions sooner —hopefully leading to better patient
outcomes, Papa said.

If approved by the Food and Drug Administration (FDA),
Papa’s test would be the first of its kind.

“We’re just a couple of years from it being available,” she
added. “We’re hoping the blood test will find injury where it couldn’t be seen
before.”

Papa noted that it was also tested in patients of all ages
with severe traumatic brain injury and seems to “cover the spectrum” of brain
injuries.

The future
of precision pathology

Dr. Michael
J. Donovan, a pathologist and researcher at The Mount Sinai Hospital in New
York, said there are many new innovations happening with blood tests.

There are
also developments in tests to evaluate urine, saliva, and cerebrospinal fluid.

In the
oncology field, for instance, researchers are looking at cell-free DNA, which
is the deoxyribonucleic acid (DNA) in blood that can be isolated and sequenced
to identify cancer-related genes with mutations in them.

Doctors then
can tell if a cancer drug will work on the patient. The blood acts as a
companion diagnostic to gauge the efficacy of a drug before it’s even given.

“That’s one
of the very practical ways in which the field has advanced,” Donovan told
Healthline.

Other blood
test innovations include evaluations that can predict how a kidney transplant
patient will respond. This can replace an invasive biopsy. Researchers are also
targeting urine tests in this area.

Donovan said
that blood tests in the future may also be able to determine if a patient is
building a response to human papillomavirus (HPV) in a lesion instead of having
to biopsy it or conduct an intrusive cell scraping.

Blood tests
are also a pathway to evaluate the possibility of neurodegenerative diseases
such as Alzheimer’s disease.

“We’re
always looking for ways to detect the manifestation of Alzheimer’s prior to
actual clinical manifestations,” Donovan said.

Other liquid
biopsy options could evaluate a patient’s immune system via bloodwork so
doctors can monitor an individual’s state via a simple blood draw.

Assessing
DNA isn’t the only research pathway, either, as researchers are looking to
evaluate peptide fragments in the blood. Peptides are amino acids that are
linked via peptide bonds and in some ways are similar to DNA and ribonucleic
acid (RNA).

DNA gives a
blueprint of genes, while RNA helps to execute the DNA’s guidelines.

Donovan said
that the field of liquid biopsies will continue to evolve — again, not just via
bloodwork but through spinal fluid, saliva, and urine. That will give doctors
multiple tools to better diagnose patients and hopefully improve outcomes.

“What we see
in blood under a microscope, that will pull together a more comprehensive
genotype of a person,” Donovan said.

“That’s the
future of precision pathology,” Donovan added.

Poor sleep – even if you don’t have sleep apnea – may be linked to higher risks of developing an irregular heartbeat. In addition, getting less rapid-eye movement (REM) sleep may also be linked to higher atrial fibrillation risks.
Disruptions in sleep may be raising your risks of an irregular heartbeat, known as atrial fibrillation (AF), according to preliminary research presented at the American Heart Association’s Scientific Sessions 2016.
Obstructive sleep apnea, sleep interrupted by pauses in breathing, is a known risk for atrial fibrillation – an irregular heartbeat that can lead to strokes, heart failure and other heart-related complications. But whether there’s a relationship between disrupted sleep and atrial fibrillation even when there’s no sleep apnea is unclear.
Presentation 733: Researchers at the University of California, San Francisco examined three sources of data – each using a different approach – to isolate and confirm the effects of poor sleep on atrial fibrillation. Their analyses of these studies showed that:
  • disrupted sleep, including insomnia, may be independently associated with atrial fibrillation;
  • people who reported frequent night-time awakening had about a 26 percent higher risk of developing atrial fibrillation compared to those who didn’t wake up a lot; and
  • people diagnosed with insomnia had a 29 percent higher risk of developing atrial fibrillation compared to those without insomnia.
  • Insomnia meant having trouble falling asleep, not getting enough sleep, or having poor sleep.

“The idea that these three studies gave us consistent results was exciting,” said lead study author Matt Christensen, currently a fourth-year medical student at the University of Michigan in Ann Arbor.
Past research has shown a link between poor sleep among people who already had AF. But this study focused on people whose pre-existing sleep disruptions were associated with developing AF later in life.
The data sources included the Health eHeart Study – an internet-based cross-sectional study of more than 4,600 people; the Cardiovascular Health Study – an 11-year longitudinal study of just over 5,700 people, of which almost 1,600 (28 percent) developed atrial fibrillation; and the California Healthcare Cost and Utilization Project, a hospital-based database spanning five years and covering almost 14 million patients.
In all three studies, researchers adjusted for the effects of obstructive sleep apnea and AF risk factors that might also be related to sleep. Some of those factors were age, sex, race, diabetes, high blood pressure, heart failure and smoking.
Diving deeper into sleep patterns and AF
Poster 218: In a separate analysis, the same researchers reviewed a subset of the Cardiovascular Health Study to understand the effect of sleep disruptions during different sleep phases without obstructive sleep apnea on atrial fibrillation risks.
The analysis showed that having less rapid-eye movement (REM) sleep than other sleep phases during the night is linked to higher chances of developing atrial fibrillation.
“By examining the actual characteristics of sleep, such as how much REM sleep you get, it points us toward a more plausible mechanism. There could be something particular about how sleep impacts the autonomic nervous system,” Christensen said. The autonomic nervous system plays a major role in controlling heart rate and blood pressure.
Another possible explanation for the link between sleep disruptions and atrial fibrillation is that frequent waking puts extra stress on the heart’s chambers, said Christensen. Participants in this analysis were also enrolled in the Sleep Heart Health Study. They had a formal sleep study to objectively measure sleep quality. That’s another element which strengthened the study’s conclusions, said Christensen, as it didn’t rely on self-reported data.
In this analysis, 1,131 people (average age 77) participated in a study with almost 10 years of follow-up.
Researchers measured how long participants slept, how well they slept, how long it took to fall asleep and the patterns of sleep (i.e., how much time was spent in rapid-eye movement (REM) sleep versus non-REM sleep). Then they analyzed the sleep disruptions’ effects to control the effects of age, sex, race, smoking, diabetes, high blood pressure, and other risk factors.
Study authors say the exact link between sleep and how AF develops is still a mystery, but we are getting closer to a clear picture.
“Ultimately, even without a clear understanding of the responsible mechanisms, we believe these findings suggest that strategies to enhance sleep quality, such as incorporating known techniques to improve sleep hygiene, may help prevent this important arrhythmia,” said senior author of both abstracts Gregory Marcus, M.D., M.A.S., a cardiologist at the University of California, San Francisco.

All three are early life factors which can be modified and the research highlights the possibility that prompt intervention could have an impact in curbing the growth in childhood overweight and obesity.
The paper, which was published in US journal Pediatrics, is the first research in the UK to look at the patterns of body mass index (BMI) weight development in the first 10 years of a child’s life and to examine the lifestyle factors that appear to predict weight gain.
Being overweight or obese is linked to a child having poorer mental health, which can extend into adolescence and adulthood. This poorer psychosocial well-being includes low self-esteem, unhappiness as well as risky behaviours such as cigarette smoking and alcohol consumption.
The research is based on the Millennium Cohort Study, a study of children born into 19,244 families in the UK between September 2000 and January 2002. Data on weight and height was collected when the children were 3, 5, 7 and 11.
This research used observational information which does not allow firm cause and effect conclusions to be drawn. However the results are based on data from thousands of children and the researchers were able to take account of many of the influences on the development of a child’s weight.
“It is well known that children of overweight or obese mothers are more likely to be overweight themselves, probably reflecting the ‘obesogenic’ environment and perhaps a genetic predisposition to gain weight,” said Professor Yvonne Kelly (UCL Epidemiology and Public Health), who led the research.
“This study shows that disrupted routines, exemplified by irregular sleeping patterns and skipping breakfast, could influence weight gain through increased appetite and the consumption of energy-dense foods. These findings support the need for intervention strategies aimed at multiple spheres of influence on BMI growth.”
Smoking in pregnancy has been linked to a higher risk of a child being overweight, possibly due to a link between foetal tobacco exposure and infant motor co-ordination which could be a developmental pathway to BMI growth.
The study identified four patterns of weight development. The large majority of children, 83.3%, had a stable non-overweight BMI, while 13.1% had moderate increasing BMIs while 2.5% had steeply increasing BMIs. The smallest group, 0.6%, had BMIs in the obese range at the age of 3 but were similar to the stable group by the age of 7.
Girls were more likely to be in the “moderately increasing” group while Pakistani, Black Caribbean and Black African children were more likely to belong to the “high increasing” group.
The research also looked at other factors to see what influence, if any, they had on children’s weight.
After taking account of background factors, breastfeeding and the early introduction of solid food were not associated with children’s weight. Likewise, sugary drink consumption, fruit intake, TV viewing and sports participation were not strong predictors of unhealthy weight gain.

That sounds like a drastically uncomplicated, unimaginative way to describe sitting nearly front row at a sneak peek of Mario Dedivanovic’s Master Class, but not even one minute into his demonstration, I had already picked up a game-changing trick to add to my own makeup routine. So, you know, the idiom rings true.

Mind you, when I received the invite for this event, I started freaking out for more that just knowing I’d leave with a better understanding of blending eyeshadow. Dedivanovic is like the makeup artist to the stars, and is also the man who made “contouring like a Kardashian” a thing. In fact, he’s Kim K.W.’s go-to man for all her beauty needs, regularly making appearances on her app in videos, how-tos, and more. Apart from being the King of the Kardashian glam squad, this year Dedivanovic launched a palette (it’s sold-out, so good luck finding it) with Anastasia Beverly Hills, and continued to reign the beauty sphere with his elusive Master Classes. Tickets to his event in Dubai cost almost $1,700.

So to get back to the point, you better bet I secured myself a good seat, recorded the entire thing, took extensive notes, and considered canceling my later plans so I could go home and try out everything he did on stage. Here’s a few tricks and interesting beauty facts I’ll now never be without.

1. Dedivanovic doesn’t use a primer, which should enthrall anyone who wants the most minimal routine possible. He told us he feels as though the primer puts a wall between the makeup and the skin and doesn’t allow it to become one. Instead, he starts off his work with an emollient moisturizer, like Kiehl’s Ultra Facial Moisturizer ($20; nordstrom.com), which grabs hold of powder and creates a natural look.

2. After moisturizing the face, Dedivanovic moves onto the body. I wouldn’t usually associate hydrating my arms and décolletage with creating a makeup look, but according to the pro, it’s important to apply a moisturizer to anywhere on the body that will be showing. So if you’re wearing a sleeve-less dress, this will help ensure your arms are luminous and reflecting light. He used Jergen’s Shea Butter Moisturizer ($9; amazon.com) on-stage to give the model moisturized, radiant-looking skin because it absorbs quickly and doesn’t feel greasy afterwards.

3. While the pro is an Instagram sensation with over 2 million followers, he hates Instagram brows, saying they give the face a “mean, witchy” look. To keep the brow looking as soft and natural as possible, he focuses on the outer corners and less on the areas closer to the eye. This will also lift and elongate the face, he explained. First he fills them in with a pencil, tops it with a powder, and sets them with either hairspray and spoolie brush (genius) or a brow gel.

4. He doesn’t use eye primer, which means yet another step I can skip in the morning. The pro explained that a cream, plus a translucent powder, like Laura Mercier’s Loose Translucent Powder ($38; sephora.com) does the same thing.

5. As if applying a precise, neat line of liner wasn’t hard enough, Dedivanovic dropped a major makeup truth bomb when he told the crowd he usually applies four (!!!) layers of eyeliner. According to the pro, after you apply your liner on top of your shadow, you’ll notice the line isn’t that dark because the shadow starts absorbing it and changes its color. So in order to make a crisp, rich liner, he applies a line of pencil, cream or gel, and the sometimes liquid on top. In our sneak peek, he started off with the Kylie Cosmetics Kyliner in Brown ($26; kyliecosmetics.com).

6. To wash his brushes, he uses baby shampoo with… wait for it… a few drops of tea tree oil to disinfect. I have never been more excited to clean my tools.

7. The pro surprisingly admitted he wasn’t that big of a fan of liquid lipsticks, which surprised me because it’s basically the only thing the Internet wanted to talk about this year. But in order to keep prevent the texture from looking dry and cracked when using them on shoots, he first exfoliates the lips with a scrub and a toothbrush.

8. When it comes to baking, there isn’t a specific time frame you have to follow, which means you don’t need to set aside 30 minutes for this technique. The pro noted that after baking for about 20-30 seconds, your concealer should be set.

9. To prevent harsh lip liner lines, which is something I always struggle with, the pro told me to really work on blending with my finger or a brush. He said often his lip looks only consist of filling the entire lip in with a liner and then a balm, so the magic is in how much pressure you’re applying with the liner and blending it out.

10. As if you needed another reason to love Beautyblender, Dedivanovic used a damp Beautyblender to kneed liquid foundation into the skin, giving the skin a fresh, glowy, luminous appearance and allowing the skin and product to become one.

Scientists have developed a urine test that measures the health of a person’s diet.
The five-minute test measures biological markers in urine created by the breakdown of foods such as red meat, chicken, fish and fruit and vegetables.
The analysis, developed by researchers from Imperial College London, Newcastle University and Aberystwyth University, also gives an indication of how much fat, sugar, fibre and protein a person has eaten.
Although the work is at an early stage, the team hope that with future development the test will be able to track patients’ diets. It could even be used in weight loss programmes to monitor food intake.
Evidence suggests people inaccurately record their own diets, and under-report unhealthy food while over-reporting fruit and vegetable intake – and that the likelihood of inaccuracies in food diaries increases if a person is overweight or obese.
Professor Gary Frost, senior author of the study from the Department of Medicine at Imperial said: “A major weakness in all nutrition and diet studies is that we have no true measure of what people eat. We rely solely on people keeping logs of their daily diets – but studies suggest around 60 per cent of people misreport what they eat to some extent. This test could be the first independent indicator of the quality of a person’s diet – and what they are really eating.”
In study, published in the journal Lancet Diabetes and Endocrinology and conducted at the MRC-NIHR National Phenome Centre, the researchers asked 19 volunteers to follow four different diets, ranging from very healthy to very unhealthy. These were formulated using World Health Organisation dietary guidelines, which advise on the best diets to prevent conditions such as obesity, diabetes and heart disease.
The volunteers strictly followed these diets for three days while in a London research facility, throughout which the scientists collected urine samples in the morning, afternoon and evening.
The research team then assessed the urine for hundreds of compounds, called metabolites, produced when certain foods are broken down in the body.
These included compounds that indicate red meat, chicken, fish, fruit and vegetables, as well as giving a picture of the amount of protein, fat, fibre and sugar eaten. They also included compounds that point to specific foods such as citrus fruits, grapes and green leafy vegetables.
From this information the researchers were able to develop a urine metabolite profile that indicated a healthy, balanced diet with a good intake of fruit and vegetables. The idea is this ‘healthy diet’ profile could be compared to the diet profile from an individual’s urine, to provide an instant indicator of whether they are eating healthily.
The scientists then tested the accuracy of the test on data from a previous study. This included 225 UK volunteers as well as 66 people from Denmark. All of the volunteers had provided urine samples, and kept information on their daily diets.
Analysis of these urine samples enabled the researchers in the current study to accurately predict the diet of the 291 volunteers.
Professor John Mathers, co-author from the Human Nutrition Research Centre at Newcastle University, said: “For the first time, this research offers an objective way of assessing the overall healthiness of people’s diets without all the hassles, biases and errors of recording what they’ve eaten.”
The team now hope to refine the technology by testing it on larger numbers of people. They also need to further assess the accuracy of the test on an average person’s diet, outside of a research setting.
Dr Isabel Garcia-Perez, co-author from the Faculty of Medicine at Imperial explained: “We need to develop the test further so we can monitor the diet based on a single urine sample, as well as increase the sensitivity. This will eventually provide a tool for personalised dietary monitoring to help maintain a healthy lifestyle. We’re not at the stage yet where the test can tell us a person ate 15 chips yesterday and two sausages, but it’s on the way.”
The team added the technology may one day be used alongside weight loss programmes, as well as patient rehabilitation, for instance to help heart attack patients follow a healthy diet.
Professor Elaine Holmes, co-author from the Department of Surgery and Cancer at Imperial added: “We are hoping to make this test available to the public within the next two years. The idea would be to collect a urine sample at home and deliver it to a local centre for analysis. We envisage the tool being used by dieticians to help guide their patients’ dietary needs, or even by individuals who are interested in finding out more about the relationship between diet and their health”
Dr Des Walsh, head of population and systems medicine at the Medical Research Council said: “Though this research is still in its early stages, it’s grappling with essential methods in food and diet studies where advances are really needed. Measuring what we eat and drink more accurately will widen the benefits of nutrition research, developing better evidence-based interventions to improve individual’s health and reduce obesity.”
Professor John Draper, co-author from Aberystwyth University added: “The future challenge is to apply the technology developed in this laboratory study in a community setting and objectively monitor diet in the home. The teams in Aberystwyth and Newcastle have been doing just this and the results are looking very promising.”
The work was funded by the Medical Research Council and the National Institute for Health Research.

Currently, the main competitor for Amazon in the smart home speaker market is Google which has followed up its release of its Google Home speaker with a push into smart home technology.

It’s expected that Apple is on the cusp of making a dramatic entrance into the market by launching its own home speaker feature voice assistant Siri, as early as next month.

While there will no doubt be a lot of excitement over such an announcement, Amazon may have just taken some wind out of Apple’s sails with the release of its new-look device.

That’s because according to Apple executive Phil Schiller in an interview last week, any Apple home speaker device would likely come with a screen.

“There is a lot of talk in the industry about voice-driven assistants and we believe deeply in voice-driven assistants that’s why invest in Siri,” he said.

But the senior vice president of worldwide marketing at Apple said in terms of user experience, he didn’t think such products functioned optimally without a screen.

“There’s many moments where a voice assistant is really beneficial, but that doesn’t mean you’d never want a screen. So the idea of not having a screen, I don’t think suits many situations,” he said.

But what could’ve been a valuable point of difference for Apple has now disappeared.

According to reports this week, Apple employees are already testing out the Siri speakers in their homes.

The new Echo Show goes on sale in the US on June 28 for $US230 ($A312) making it the priciest Echo yet.

Have you ever had to come out of a high plank not because your core was on fire—but because your wrists were? Certain strength moves put a lot of pressure on your forearms. There’s good news for the sore-wristed though: Simple adjustments and smarter stretching can help you avoid that annoying ache.

“Wrist pain and discomfort during [a workout] is likely due to improper wrist positioning, overloading weak structures, or overuse,” explains Paul Mostoff, the chief of physical therapy at All Sports Physical Therapy in New York City. “The tendons of the wrist can become hot, painful, inflamed, swollen, and degenerated over time—and trying to work out through the pain can turn a simple acute condition, like tendonitis, into something more serious and chronic.”

Take a basic exercise like a push-up: “It keeps the wrist in an extended position while loading your bodyweight through that structure, which will increase the pressure through the carpal tunnel and the wrist joint,” Mostoff says. “Do this for several sets, several times per week, and you open yourself up for some wrist discomfort and irritation, especially if your wrists aren’t accustomed to that type of exercise.”

Other moves that can lead to sore wrists include bench presses, squats while cradling a barbell or free weights, and bicep curls with poor form.

It’s also possible that wrist pain is linked to a strength or mobility issue somewhere else, like the shoulders. “The wrists and forearms might be taking more abuse during a workout because the shoulder joint lacks mobility, so the forearm muscles compensate,” says Mostoff.

If you suffer from exercise-induced soreness, read on for Mostoff’s tips on easing the pain; plus his advice on strengthening your wrists so they can power through your entire resistance routine.

How to treat wrist pain

The first step (of course) is to stop doing any exercise that irritates the wrist. “You’ve got to cut off the inflammation and [keep] any further damage from occurring,” says Mostoff.

To help your wrists heal, Mostoff recommends applying ice to the area daily, and gently massaging the muscles in the forearm to reduce tension. You might also consider wearing wrist splints to take pressure off the joints, he adds.

While the pain persists, avoid doing any stretches that put the wrist in a bent position (like Downward Dog). But once the inflammation has calmed down, stretching the forearms is another way to help release tension: “Bend the wrist up while keeping the arm and elbow straight and hold for 30 to 60 seconds,” Mostoff says. “Reverse the motion and bend the wrist down and hold for another 30 to 60 seconds.” To deepen the stretch, use the opposite hand to pull gently on your fingers.

Once you’re ready to get back to your strength training routine, you can protect your wrists by wrapping them with athletic tape or wearing a wrist strap. Also make sure you always maintain a neutral wrist position. “For example, if you’re doing bicep curls, make sure your wrist is completely straight as you curl the weight towards your body,” Mostoff says. (Can’t keep your wrist aligned? You may be using too much weight.) “If you’re doing push-ups, use parallel bars or do the push-up on your knuckles so your wrists are straight while you perform the exercise,” he adds.

How to strengthen your wrists

“The muscles that control the wrist are actually in the forearm,” says Mostoff. You can target those muscles with what’s called resisted wrist flexion and extension, and pronation and supination. Here’s how to do it:

While seated on a bench, grab a dumbbell with an underhand grip (palms facing up), and rest your forearm on your thigh with your wrist hanging off your knee. Allow the dumbbell to lower as far as possible while you relax the wrist. While keeping the forearm still, raise the dumbbell back up as high as possible. Lower slowly and repeat.

“You can also perform this exercise with your palms facing down to reverse the motion and strengthen the opposing muscle groups,” Mostoff says.

Another great wrist-strengthening exercise is farmer’s walks: Stand tall while holding a pair of dumbbells or kettlebells on either side of your body; palms facing in. Then walk 50 to 100 feet. “Carrying the weight will help improve your overall grip strength and muscular endurance,” Mostoff explains.

Finally, you can strengthen the muscles in your hands, which also support your wrists, by playing with silly putty. Roll, squeeze, and spread the putty with your fingers for several minutes a day.

When to see a doctor

Call your MD or see a physical therapist if your wrist swells up, or the pain is sharp or severe, says Mostoff. Another sign it’s time to seek help: The pain lasts more than one to two weeks, and doesn’t improve with ice, rest, and OTC pain relievers like nonsteroidal anti-inflammatory drugs.

Milli Hill, a British mother and writer, invited a few women
to her house for tea and cake to talk about giving birth.

A “positive
birth” can leave you with your child feeling “on top of the world, satisfied,
and strong,” Hill told Healthline.

But at that
first gathering and elsewhere, she heard stories that too often included
comments like, “They did not let me,” or “I was not allowed.”

Why weren’t mothers having the labors and deliveries they
wanted?

Maybe support groups for pregnant women that focused on
empowering them would make a difference, she thought.

In a few weeks, she received more than 100 emails from women

Hill believes that a “climate of fear” has affected not just

Caroline Handschuh, a New

Samantha Drury was inspired to take charge by an unhappy
first birth. To bear her eldest, she had to stay in bed for 12 hours, hooked up
to fluids, using a bed pan and going without solid food.

Restrictions are common when women have preeclampsia or
diabetes. Drury had no complications. Still, she didn’t protest.

“I thought you do what the doctor said and my being happy

Three years later, for her next birth, she hired a doula and

doula was absolutely wonderful.”

Drury began studying online to become a doula a month later.

monthly Positive Birth group. She has now been present for 30 births.

In about a third of those births, women were confined to
their beds as she had been, despite no complications.

“I remind the mother that you can say that you want to get
up,” she said.

Getting up will ease the labor, Giuditta Tornetta, another
doula, told Healthline, as long as the woman is healthy, and her blood pressure
and the baby’s heart beat are normal.

“It’s much less painful while you’re walking or standing up
or squatting,” she said.

But that freedom poses a practical problem for overstretched
nurses, Tornetta explained.

“It’s 3:00 in the morning, I have three other patients, I
hook her up to the monitor so I can leave the room,” she said.

The concern is you might fall, if you’re not in bed, and

Finding joy in giving birth

Tornetta founded the Joy in Birthing Foundation, which
matches volunteer doulas with about 200 low-income women a year in and around
Los Angeles.

charge can leave them free to move.

Women helping each other — that’s the message of the
fast-growing positive birth movement. Women are advised to look for a doctor

want women to have positive experiences,” Dr. Jeffrey L. Ecker, chief of the

Those are
questions being discussed intently at an annual conference in California.

A research
team, led by William Mandy, PhD, senior lecturer in clinical psychology at
University College London, says it has gained new insights into the different
ways that autistic characteristics present themselves in girls during
adolescence.

Mandy
presented the findings today at the 16th Annual International Meeting for Autism
Research (IMFAR) in San Francisco.

The findings
are new, but they echo theories offered by Hans Asperger in 1943 that were never tested.
Asperger, a medical theorist, is known for his early work into autism spectrum
disorders.

Mandy’s team
conducted a longitudinal study, which repeatedly gathered data for the same
test subjects over a period of time.

Researchers
found that while boys tend to display stable, similar autistic characteristics
throughout their adolescence, girls are more likely to see these
characteristics ramp up during the teen and preteen years.

The findings
could help explain why boys tend to be diagnosed with autism earlier than girls,
and also how guidelines for diagnosing autism in children could be biased against
girls.

1. A difficult disorder to pin down

Autism is
not the easiest condition to diagnose.

“Unlike some
physical health difficulties, we don’t have a biomarker for autism,” Mandy told
Healthline. “We don’t have blood tests or brain scans. We can’t actually see
autism itself, so instead we do what people do in all mental health disorders,
pretty much. We diagnose it not by looking at the thing itself, but by looking
at its manifestation, its size, and its symptoms.”

In short,
diagnosing autism isn’t quite an exact science. The criteria for diagnosing
autism includes a group of observable characteristics and behaviors that the
medical community has come to a consensus on as representing autism.

Generally
speaking, these characteristics come down to difficulties in the realm of
social communication and flexibility when it comes to things like switching
activities and focus. Other autistic characteristics include sensitivity to
outside stimuli like bright lights or loud noises.

“Autism is
not a black and white thing,” said Mandy. “It’s a dimensional condition. So the
people that we label as having autism are really just at the extreme end of a
continuum that extends all the way through the population with no clear natural
cut point between those that have autism and those that don’t. And what’s
become clear from the research is that having autistic traits, even if they’re
not at the level where we would conventionally label someone as having a
clinical diagnosis of autism, that’s still a risk factor for a range of
difficulties. For example, developing social anxiety problems, conduct problems,
or anorexia.”

2. Research is first of its kind

Mandy’s team
looked at autistic traits in the general population, rather than limiting
itself just to those people who are at the extreme end of the spectrum.

Autistic
traits for the same group of children and adolescents was measured at ages 7,
10, 13, and 16.

Boys who
showed high levels of autistic traits at age 7 tended to remain consistent over
time, demonstrating similar traits at older ages.

Girls, on
the other hand, showed a marked increase in levels of autistic social
difficulties between the ages of 10 and 16.

Mandy said
the findings were surprising, as previous medical wisdom stated that girls and
women with autistic traits tended to “camouflage” them as they got older.

“If
anything, I expected to see a decline in autistic symptoms in girls over time,”
he said. “What’s very interesting is that there was one person who suggested
the opposite, and that was Hans Asperger himself. There’s this rather
intriguing sentence from this paper he wrote in the 1940s, where he’s wondering
why we never see girls with what he would call ‘autistic psychopathologies.’
And he said, ‘Well, maybe it’s because these traits don’t show onset until
adolescence with females.’ And nobody tested that idea. So it’s intriguing that
that would be what we appear to have found on this occasion.”

3. Possible diagnostic pitfalls

So, are
girls getting short-changed when it comes to autism diagnoses?

“It’s
possible — it’s likely, in fact — that our current diagnostic criteria are
rather biased toward the male presentation, and biased against the female
presentation, said Mandy. “And there’s always been a sort of circular
situation, that almost all autism research is done on males, which means that
your diagnostic criteria reflects males, which means that you can continue to
recruit a predominance of males in your research, and so it goes on.”

Besides this
apparent bias, there’s also a strong likelihood that girls with autism present autistic
characteristics in ways that are different – and subtler – than what are seen
in boys.

One
characteristic of autism, that holds true with both sexes, is a strongly
focused interest on a particular topic.

Where the
sexes often differ, says Mandy, is in the nature of this interest.

“There’s
emerging evidence, and this certainly fits with my clinical impression, that
girls with autism, their special and focused interests, are a little bit
unusual than autistic boys,” he said. “They’re less likely to focus on
something technical and specific, and perhaps more likely to focus on the
social realm.”

So while a
boy with autism might show a preoccupation with something technical like trains
or buildings, a girl with autism is more likely to focus on hierarchies or
lists of family and friends.

“Often,
girls are more likely to be almost stereotypically gender-specific,” said
Mandy. “So you meet a lot of autistic girls who are really into animals or
horses, or fashion. And those interests, of course, don’t jump out at you as
much. If you get a kid who comes along and says, ‘I’m obsessed with the
District Line on the London Underground,’ then that looks unusual, and you
think autism might be an issue. If you have a girl who says, ‘I’m obsessed with
wearing the latest styles,’ that obviously doesn’t seem as unusual, so it’s
less likely to alert people to the presence of autism.”

Mandy also
points out that the way girls’ autistic traits seem to accelerate between the
ages of 10 and 16 mirrors a changing and complex social world.

“I think for
girls, there’s a phenomenon where they can be doing fine at primary education,”
he explained, “but as the social world starts to become more complex, as they
transition over into secondary school and the social demands of the adolescent
female social world rapidly accelerate, these girls can really struggle, and
people often don’t understand.”

4. Support always key

While
changing guidelines to reflect traits of autism in girls seems like an obvious
partial solution, it’s really not that simple.

Because
autism exists on a spectrum and, as Mandy tells us, it’s not a black-and-white
diagnosis, changing diagnostic guidelines could shift the focus too much.

“I think the
way to go is to keep the same fundamental diagnosis,” Mandy said. “Fundamentally, this is about difficulties with social communication, a tendency toward inflexibility, but I do think that people need to be more flexible in thinking in terms of how these manifest and whether the way these manifest in girls and women — especially girls and women with a normal range IQ — is a bit different.”

People with autistic traits can thrive, but it’s crucial that their needs are recognized and that they’re placed in an environment where they’re able to excel.

“I think we
need better understanding of the early presentation of autism in girls, so we
can identify them in a timely way, and for those that need help, we can put
that support in place before things start to go wrong in adolescence,” said
Mandy. “I think we need to get better clinically at thinking dimensionally, and
not just thinking in these black-and-white terms. Trying to understand people
in a more subtle way, and thinking about if they have traits or conditions for
autism diagnosis, those are important.”

Mandy says
that now that his team has gleaned some new insights into autism in girls,
they’d like to go more in-depth in order to better understand the condition.

“I think
what we need to do now is look at it in a bit more depth. Who are these girls
who are apparently not showing autistic traits in childhood, and who are
showing them in adolescence?” he said. “And asking questions like, ‘Are these
social difficulties actually autistic in nature, or are they arising from
something else?’ If they are autistic in nature, what were the early indicators
that were being missed by this measure of autistic traits in childhood? So,
it’s really about trying to get a more detailed picture so we can properly
understand the meaning of this finding.”
